We work with skilled nursing facilities to maintain continuity of care by providing outpatient services to residents discharged to home. These healthcare services are selectively provided to patients who are still able to benefit from ongoing rehabilitation services. Depending on state regulations, space and access requirements, it is preferable for residents to maintain clinical routines with the same therapist when possible.
